Formation, conseils et extraction: les defis poses par le Norplant.

The contraceptive implant system Norplant is a very promising option because it is effective, reversible, and long-lasting. It is not, however, a perfect solution. Health personnel must undergo training to master the technique of Norplant insertion and removal. The Association for Voluntary Surgical Contraception, Family Health International, the Population Council, and the Program for Appropriate Technologies in Health have conducted such training in Nigeria, Bangladesh, and Kenya. Health workers must also be trained in the logistics of Norplant and sterile equipment needed to insert and remove the capsules. In fact, distribution of Norplant must be improved. Problems of distribution and cost assume crucial importance. Thailand charges high custom duties on Norplant, which makes it much more costly than it is in Indonesia. Norplant use is focused in the southern provinces of Thailand, where sterilization is not a largely accepted family planning method. Perhaps the most important element of training is client counseling, especially providing them clear information at each step of the Norplant acceptance process. Family planning providers must understand that women have the right to have Norplant removed on demand. Problems with follow-up of clients are linked to those of access to removal. Specific needs of each country complicate training in Norplant insertion and removal, counseling services, access to removal, and follow-up of Norplant clients. Sterilization is forbidden or perceived negatively in many countries, so Norplant can be an advantageous solution to women who do not want any more children. As medical and family planning environments prepare themselves for the introduction of Norplant, it is proper to diligently busy themselves in applying knowledge learned from clinical tests of distribution systems of both public and private medical services.
