Alternative definition : MMHCC: High grade based on marked nuclear atypia, including macronucleoli, marked enlargement with hyperchromasia and irregular nuclear contours, abundant mitoses/apoptosis, luminal comedo necrosis.; MMHCC: mPIN which has shown documented progression to invasive cancer.; MMHCC: Qualifier of local vs. extensive/diffuse can be added, similar to that described above for atypical hyperplasia. Focal as one to several foci, less than 50% of glands involved. Extensive/diffuse indicating involvement of greater than or equal to 50% of glands.; MMHCC: In human, HGPIN occurs in previously normal prostate, so epithelial proliferation occurs within preexisting basement membrane bound glands and ducts. In transgenic mouse, creation of apparently increased glandular spaces compared to wild type mice is not necessarily indicative of invasive carcinoma. If such a lesion occurs uniformly throughout the prostate, for example, with lobular expansion by connecting large and small duct profiles (with similar cytologic atypia), and a symmetric lower power architecture, this should likely be considered an in situ process (e.g., images LPBTag12T5-7689DLP(10x); TRAMP538(10x); LPBTag12T7f-7874DLP(10x)). This concept is particularly evident when architecturally distinct foci of unequivocal stromal or periprostatic fat invasion (of small crowded glands, single cells, or nests of cells) occur against this more uniform symmetric lobular expansion HGPIN background.;
