NCIt definition : A form of palliative care where all patients receive care for their condition, but
with a minimum of required contact with specialty trained clinician and more intensive
treatment reserved for those who do not benefit sufficiently from the less intensive
treatments. A key element of this model is that patients must be monitored systematically
and stepped up to more intensive treatment if the minimal level of care does not achieve
sufficient health gains.;
