measures include: 1) preoperative verification of patient identity, procedure and
site; 2) marking the operative site; and 3) taking a time-out just prior to starting
the procedure during which surgical team members communicate about and confirm patient
identity, site and procedure, and resolve any other relevant questions or concerns.
