After the patient ingests a dose of fructose, any carbohydrate that is not absorbed is metabolized by intestinal bacteria During this process, hydrogen gas is produced, which is absorbed and eventually exhaled through the lungs; the amount of hydrogen in the expired gas is measured at various intervals depending on the protocol being used.
