occasionally moist ( 2) or rarely moist ( 1) in the Neonatal Skin Risk Assessment
Scale (NSRAS). Constantly moist skin is moist/damp every time the infant is moved
or turned. Moist skin is often damp but not always; linens must be changed at least
once a shift. Occasionally moist skin requires an extra linen change approximately
once a day. Rarely moist skin is usually dry; linens require changing only every 24
hours. Only one numeric value is selected.;
